tcp: improve SEG.ACK validation

Implement the improved SEG.ACK validation described in RFC 5961.
In addition to that, also detect ghost ACKs, which are ACKs for data
that has never been sent.
The additional checks are enabled by default, but can be disabled
by setting the sysctl-variable net.inet.tcp.insecure_ack to a
non-zero value.

tcp: add counter to track when SACK loss recovery uses TSO

Add a counter to track how frequently SACK has transmitted
more than one MSS using TSO. Instances when this will be

netinet: add a probe point for IP, IP6, ICMP, ICMP6, UDP and TCP stats counters

When debugging network issues one common clue is an unexpectedly
incrementing error counter. This is helpful, in that it gives us an
idea of what might be going wrong, but often these counters may be
incremented in different functions.

Add a static probe point for them so that we can use dtrace to get
futher information (e.g. a stack trace).

For example:
dtrace -n 'mib:ip:count: { printf("%d", arg0); stack(); }'

This can be disabled by setting the following kernel option:
options KDTRACE_NO_MIB_SDT

tcp: retire TCPDEBUG

This subsystem is superseded by modern debugging facilities,
e.g. DTrace probes and TCP black box logging.

We intentionally leave SO_DEBUG in place, as many utilities may
set it on a socket. Also the tcp::debug DTrace probes look at
this flag on a socket.

sys: general adoption of SPDX licensing ID tags.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.

The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

No functional change intended.

Use estimated RTT for receive buffer auto resizing instead of timestamps

Switched from using timestamps to RTT estimates when performing TCP receive
buffer auto resizing, as not all hosts support / enable TCP timestamps.

Disabled reset of receive buffer auto scaling when not in bulk receive mode,
which gives an extra 20% performance increase.

Also extracted auto resizing to a common method shared between standard and
fastpath modules.

With this AWS S3 downloads at ~17ms latency on a 1Gbps connection jump from
~3MB/s to ~100MB/s using the default settings.
