Add NTB driver for PLX/Avago/Broadcom PCIe switches.
This driver supports both NTB-to-NTB and NTB-to-Root Port modes (though the second with predictable complications on hot-plug and reboot events).

This driver supports both NTB-to-NTB and NTB-to-Root Port modes (though the second with predictable complications on hot-plug and reboot events). I tested it with PEX 8717 and PEX 8733 chips, but expect it should work with many other compatible ones too. It supports up to two NT bridges per chip, each of which can have up to 2 64-bit or 4 32-bit memory windows, 6 or 12 scratchpad registers and 16 doorbells. There are also 4 DMA engines in those chips, but they are not yet supported.
While there, rename Intel NTB driver from generic ntb_hw(4) to more specific ntb_hw_intel(4), so now it is on par with this new ntb_hw_plx(4) driver and alike to Linux naming.

This means that FPU is always initialized and handled when available, and SSE+ register file and exception are handled when available. This makes the kernel FPU code much easier to maintain by the cost of slight bloat for CPUs older than 25 years.
CPU_DISABLE_CMPXCHG outlived its usefulness, see the removed comment explaining the original purpose.

Move the ConnectX-3 and ConnectX-2 driver from sys/ofed into sys/dev/mlx4 like other PCI network drivers. The sys/ofed directory is now mainly reserved for generic infiniband code, with exception of the mthca driver.
- Add new manual page, mlx4en(4), describing how to configure and load mlx4en.
- All relevant driver C-files are now prefixed mlx4, mlx4_en and mlx4_ib respectivly to avoid object filename collisions when compiling the kernel. This also fixes an issue with proper dependency file generation for the C-files in question.
- Device mlxen is now device mlx4en and depends on device mlx4, see mlx4en(4). Only the network device name remains unchanged.
- The mlx4 and mlx4en modules are now built by default on i386 and amd64 targets. Only building the mlx4ib module depends on WITH_OFED=YES .

Make CloudABI work on i386.
Copy over amd64's cloudabi64_sysvec.c into i386 and tailor it to work. Again, we use a system call convention similar to FreeBSD, except that there is no support for indirect system calls (%eax == 0).
Where i386 differs from amd64 is that we have to store thread/process entry arguments on the stack instead of using registers. We also have to put an extra pointer on the stack for TLS (for GSBASE). Place that pointer in the empty slot that is normally used to hold return addresses. That seems to keep the code simple.

In addition, it does not use bus_space but instead uses bcopy with volatile pointers triggering a host of warnings. (if_ie.c is one of 3 files always built with -Wno-error)
