The Software Package Data Exchange (SPDX) group provides a specification to make it easier for automated tools to detect and summarize well known opensource licenses. We are gradually adopting the specification, noting that the tags are considered only advisory and do not, in any way, superceed or replace the license texts.

This commit adds full support for USB 3.0 devices in host and device mode in the USB core. The patch mostly consists of updating the USB HUB code to support USB 3.0 HUBs. This patch also add some more USB controller methods to support more active-alike USB controllers like the XHCI which needs to be informed about various device state events.
USB 3.0 HUBs are not tested yet, due to lack of hardware, but are believed to work.
After this update the initial device descriptor is only read twice when we know that the bMaxPacketSize is too small for a single packet transfer of this descriptor.

I am not sure if there is a standard for having a leading and/or trailing _ in the macro name, the usb code seems to use both.
There are still several unprotected headers here so it might be useful to do the same thing on other files as well as the need arises.

Rather than losing, do what Windows does: just request the maximum size, and allow a shorter response. Obsoletes the need for UQ_NO_STRINGS, and therefore these "quirks" are removed.
usb_subr.c (1.116), author: mycroft In the "seemed like a good idea until I found the fatal flaw" department... Attempting to read a maximum-size string descriptor causes my kue device to go completely apeshit. So, go back to the original method, but allow the device to return a shorter string than it claimed.

Add preliminary support for isochronous to the UHCI driver (not for OHCI yet).
usb.c, uhci.c, ohci.c - Initial attempt at detachable USB host controllers - Handle the use_polling flag with a lttle more care and only set it if we are cold booting.
usb.c, uhci.c ohci.c, usbdi.c usbdi_util.c usb_subr.c - Make sure an aborted pipe is marked as not running. - Start queued request in the right order. - Insert some more DIAGNOSTIC sanity checks. - Remove (almost) unused definitions USBD_XFER_OUT and USBD_XFER_IN.
usb.c, usb_subr.c - Add an event mechanism so that a userland process can watch devices come and go.
ohci.c - Handle the case when a USB transfer is so long that it crosses two page (4K) boundaries. OHCI cannot do that with a single TD so we make a chain.
ulpt.c - Use a bigger buffer when transferring data. - Pre-allocate the DMA buffer. This makes the driver slightly more efficient. - Comment out the GET_DEVICE_ID code, because for some unknown reason it causes printing to fail sometimes.
usb.h - Add a macro to extract the isoc type. - Add a macro to check whether the routine has been entered after splusb and if not, complain.
usbdi.c - Fix a glitch in dequeueing and aborting requests on interrupt pipes. - Add a flag in the request to determine if the data copying is done by the driver or the usbdi layer.
