Replace two remaining hex values for control codes with their names.

These were the only values in the range from 0 to 0x1f that were defined
as hex values, all other occurances have been converted

vt: add Ctrl+/ key mapping

Copy country-code .iso syscons keymaps for vt(4)

Existing syscons ISO 8859-1 keymaps (??.iso.kbd) are usable without
change as Unicode keymaps for vt(4).
